ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join their rapidly expanding team. The PCP is accountable for outcomes, clinical care, and cost-effectiveness for a panel of up to 450 patients, acting as a health coach to help patients set and achieve health goals. The role involves simplifying care actions, working towards performance metrics such as patient admissions, appointment slots, patient experience, clinical gap closures, and medical cost measures. The PCP will function independently and collaboratively with healthcare professionals, ensuring compliance with guidelines and participating in risk and quality management programs. Responsibilities include providing care for acute and chronic illnesses in older adults, managing patient care and outcomes, coordinating with hospitalists, leading care teams, engaging with transitional care teams for patients unable to visit the office, and participating in center management and recruitment. The PCP must demonstrate availability, service orientation, evidence-based medicine knowledge, leadership, quality focus, influence, self-care, proficiency in Microsoft Office, and willingness to travel locally and nationally for training and recruiting. Education and experience requirements include an MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty, state medical license, board eligibility or certification, DEA number, and Basic Life Support certification within 90 days of employment.
